During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 10/15/2025. The on-site inspection began at 11:10AM and ended at 12:35PM. The inspector reviewed compliance in the areas listed above. There were 10 children present and 2 caregivers; the point total was 31. The inspector reviewed 5 children?s records, 2 caregiver records, and 2 household members records on-site. This inspection included: document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 10/22/2025. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-800-280
Potentially poisonous substances, materials and supplies such as, but not limited to, cleaning agents and disinfectants shall be stored away and inaccessible to children.

A container of disinfectant wipes with warning labels, ?Keep out of reach of children,? were on the shelf in the bathroom, within reach of children.

The basement storage closet was unlocked, with cleaning supplies and laundry detergent stored on the floor. The provider acknowledged children use the basement area for indoor play.
Plan of Correction: Disinfectant wipes were removed and placed in a locked cabinet during inspection.

A lock will be installed on the closet by 10/20/2025.
Standard 8VAC20-800-430-A
Children under two years of age and children over two years of age who are not developmentally ready to climb or descend stairs without supervision shall not have access to stairs.

The stairs leading to the second floor, did not have an operable gate to prevent children from accessing the stairs.
Plan of Correction: i have bought a new operable gate and it will be installed 10/20/2025.
