Inspector Notes

An unannounced monitoring inspection was conducted on 10/03/2023. There were 22 children, ages 4 months- 4 years, and 5 staff present during the inspection. The inspector reviewed compliance in the areas of administration, staff qualifications and trainings, staffing and supervision, programs, physical plant, emergencies and nutrition. A total of 5 children?s records and 5 staff member records were reviewed. The children were observed during breakfast, in free choice play and in outdoor large motor play. The inspector arrived for the inspection at 7:55 am and departed the center at 10:00 am.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Standard 8VAC20-780-160-C
Based on record review, the center failed to ensure that staff shall submit documentation of a negative tuberculosis screening at least every two years from the date of the initial screening or testing.

Evidence: The most recent tuberculosis screening in the record for Staff 5 was dated 09/01/2021; there was not a follow-up tuberculosis screening available for review.
Plan of Correction: An appointment was made during the inspection.
Standard 8VAC20-780-500-A
Based on observation and staff interviews, the center failed to ensure that children?s hands shall be washed with soap and running water after toileting and any contact with blood, feces, or urine.

Evidence: Child 6 was observed during a diaper change. Child 6 did not wash their hands with soap and running water after the diaper change.
Plan of Correction: Will purchase a stool to help children reach the sink to ensure hands are washed after diapering.
Standard 8VAC20-780-530-A
Based on record review and discussion with staff, the center failed to ensure that at least one staff in each classroom or area where children are present shall have a current certification in CPR and a current certification in first aid.

Evidence: Staff 1 was the only staff member in the 2 year old classroom. Staff 1 does not have a current CPR/1st Aid certification.
Plan of Correction: Will find a class.

Based on observations and staff interviews, the center failed to ensure that tables and high chair trays shall be sanitized before and after each use for feeding.

Evidence: The tables in the 2 year old classroom were not sanitized before breakfast. Staff 5 verified that the tables were not sanitized before breakfast.
Plan of Correction: Will remind staff to sanitize tables before and after meals.
