Inspector Notes

An unannounced, on-site focused inspection was initiated on 02/06/2025 and completed on 02/06/2025, as part of the probationary period. The Department issued a Special Order to place the licensee on probation for a period of one month and to require the licensee to contact parent, guardians, or other responsible persons regarding health and safety violations. The on-site inspection began at 2:00 pm and ended at 3:15 pm. The inspector reviewed compliance in the areas listed above. There were 20 children present and 5 staff. The inspector reviewed 5 children?s records on-site on 02/06/2025.

The inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

For each group of children, the center shall maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurs.

There were 7 children observed in Classroom 4. The written attendance for Classroom 4 documented 6 children in care.
Plan of Correction: The program director will verify attendance at the 9:00 am arrival cut-off each day.
