An unannounced renewal inspection was conducted today.
There were 10 children care with two caregivers for a total of 25 points.
The children were playing in centers and eating lunch.
Three child records and four staff records were reviewed.
We discussed the work on the new drainage ditch. You reported that the children are not playing in that area until the ditch is repaired.

Based on a tour of the physical plant, the facility failed to ensure that all areas are maintained in a safe condition.
Evidence:
1. There is a wooden shelf in the play area. It is approximately three feet high and five feet long. It is not secured to the floor and presents a tipping hazard.
Plan of Correction: The provider reported that the shelf will be stabilized to ensure it cannot be turned over.
