During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 11/19/2024 and completed on 11/19/2024. The on-site inspection began at 2:18pm and ended at 5:15pm. The inspector reviewed compliance in the areas listed above. There were 107 children present and 11 staff.

The inspector reviewed nine children?s records and eight staff records on-site.This inspection included document review, tour of the facility, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 11/29/2024. A POC submitted after this date will not appear on the public website.

Physical examinations completed by a physician shall be obtained prior to the first day of attendance or within 30 days after the first day of attendance.

There was no physical examination in the record of child #7, who had been enrolled for seven months.

The center shall ensure that areas and equipment of the center, inside and outside, are maintained in a clean, safe and operable condition.

On the playground where children were playing, there were three missing bolts from the wooden plank of the gaga pit, resulting in broken and protruding siding.

The center?s license is for ages 5 years through 12 years 11 months. Child #7 (age 4) was enrolled at the center for seven months.
