Inspector Notes

A monitoring inspection was conducted on 10/25/2022. There were 12 children present, totaling 30 points, with 2 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 2 caregiver records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and a violation was documented on the violation notice issued to the program. The violation was reviewed with the provider during the inspection.

Based upon review of caregiver records and provider interview, the provider has not ensured that a caregiver completes a sworn disclosure statement or affirmation at least every five years.
