The department received a self-reported incident regarding allegations in the areas of forbidden action, and a subsequent inspection was initiated and concluded on September 29, 2022. The licensing inspector was onsite from approximately 9:30am until 11:15am. The inspector reviewed documentation, interviewed staff and conducted observations.

The evidence gathered during the investigation did not support the self-report of non-compliance with standards or law.
