Inspector Notes
The department received a complaint regarding allegations in the areas of forbidden action and a
subsequent inspection was initiated on July 28, 2022. The licensing inspector was onsite September 27, 2022 from approximately 9:30am until 11:15am. The licensing inspector reviewed documentation, conducted observations and interviewed staff.
The evidence gathered during the investigation supported the allegation of non-compliance with standards or law, and violations were issued.
Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and returned it to me within five business days from today. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s).

Violations
1Evidence:
1. Staff #1 stated on July 18, 2022, Child #1 had a ?blow-out bowel movement?. Staff #1 stated
?some of the poop was inside the vagina of [Child #1]?. Staff #1 stated she called Staff #2 to the room to ask about the best way to clean the area.
2. Staff #2 stated she took a picture of the vaginal area of Child #1 in order to show the parent and to ensure Child #1 was cleaned properly.
3. The parent of Child #1 stated Staff #2 showed pictures of her child?s private area and was
concerned about pictures existing of the child?s vaginal area and insisted on Staff #2 deleting the photos.
4. Staff #2 stated all pictures of Child #1 were deleted.

3. Director to inform all staff of this restriction.
