Inspector Notes
A Subsidy Health and Safety Inspection was conducted on Wednesday, May 1, 2024. The inspector was on site from 10:10am to approximately 12:10pm. There were 53 children present and 11 staff members directly supervising. Upon arrival, the children were observed in different areas of the center. Some children were in their classrooms, in the hallways during transitions, and outside in their designated playgrounds. Staff were observed having positive interactions with the children. All areas of the facility including classrooms, hallways, the outdoor play area, and bathrooms were inspected. A total of five children's records and five staff records were reviewed.
A code compliance inspection was conducted on this date. Both inspections combined cover subsidy health and safety regulations. Only subsidy requirements that are not covered in the code compliance inspection were reviewed during the supplemental inspection. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Violations
3Evidence: 1. The CPR certification in the record of Staff #2, employed on 05/16/23, expired on 01/10/2024.
2. The record of Staff #5, employed on 10/23/23, did not contain documentation that the staff has current certification in CPR. All staff who work directly with children shall have current certification in CPR within 90 days of the date of employment.
3. During interview, management confirmed Staff #2 and Staff #5 did not have current certification in CPR as required.
Evidence: 1. The first aid certification in the record of Staff #2, employed on 05/16/23, expired on 01/10/2024.
2. The record of Staff #5, employed on 10/23/23, did not contain documentation that the staff has current certification in first aid. All staff who work directly with children shall have current certification in first aid within 90 days of the date of employment.
3. During interview, management confirmed Staff #2 and Staff #5 did not have current certification in first aid as required.
Evidence: The center's emergency response drill log was reviewed. The most recent evacuation drill was documented on 03/13/24.
During interview, a member of management confirmed the evacuation procedures were not practiced in the month of April 2024.
