Inspector Notes
An unannounced, on-site renewal inspection was initiated and completed on May 8, 2025, as a part of the licensure period. The on-site inspection began at 9:00am and ended at 1:05pm. The inspector reviewed compliance in the areas listed above. There were 69 children present and 17 staff. The inspector reviewed five children?s records and seven staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Violations
3Areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition.
On the playground, a plastic enclosed bubble located on the bridge of the slide was cracked and broken. The broken plastic created sharp edges and components that could entangle clothing and snag skin.
Hazardous substances such as cleaning materials shall be kept in a locked place using a safe locking method that prevents access by children.
A bottle of sanitizer with the warning label, "Keep out of reach of children," was unlocked in the four year old classroom within reach of children.
The playground does not have sufficient resilient surfacing under the fall zones of the playground equipment. The ground is exposed under the sliding area.
