Standard 8VAC20-780-270-A
Repeat Violation

Areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition.

On the playground, a plastic enclosed bubble located on the bridge of the slide was cracked and broken. The broken plastic created sharp edges and components that could entangle clothing and snag skin.
Plan of Correction: Management will put in a work order to fix the broken bubble on the slide. Also, playground safety checklist will be reviewed carefully to address hazards on the playground.
Standard 8VAC20-780-280-E
Repeat Violation
Hazardous substances such as cleaning materials shall be kept in a locked place using a safe locking method that prevents access by children.

A bottle of sanitizer with the warning label, "Keep out of reach of children," was unlocked in the four year old classroom within reach of children.
Plan of Correction: All staff will be retrained on hazardous substances being locked and out of reach of children when not in use. Moving forward, management will spot check classrooms to ensure that hazardous substances are locked up.
Standard 8VAC20-780-330-B
Where playground equipment is provided, resilient surfacing shall comply with minimum safety standards.

The playground does not have sufficient resilient surfacing under the fall zones of the playground equipment. The ground is exposed under the sliding area.
Plan of Correction: A work order will be placed to receive mulch for resilient surfacing. In addition, management will review playground safety procedures and checklist to ensure that the resilient surfacing is maintained.
