Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on November 20. 2025, as a part of the licensure period. The on-site inspection began at 9:00am and ended at 10:50am. The inspector reviewed compliance in the areas listed above. There were 38 children present and 10 staff. The inspector reviewed five children?s records and five staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

The center is required to obtain background checks from any state in which the individual has resided in the preceding five years.

Staff #2, employed for one month, did not have documentation of a sex offender registry check from one state.
Plan of Correction: A sex offender registry search was conducted at the time of the inspection and yielding no results. Staff #2 was never left alone with children and was always supervised by another staff member with qualifying background checks. Moving forward, management will ensure to conduct a sex offender registry search for any individual that has lived in another state in the preceding five years.
