Inspector Notes

An unannounced monitoring inspection was initiated and completed on February 11, 2026 in response to a self-report received by the licensing office on February 9, 2026 relating to forbidden actions. The inspector reviewed three children?s records and four staff records on-site. This inspection included document review and interviews.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on February 19, 2026. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-400-B
Behavioral guidance shall be constructive in nature, age and stage appropriate, and shall be intended to redirect children to appropriate behavior and resolve conflicts.

Staff #2, 3, and 4 reported during interviews that Staff #1's tone was not appropriate with children. Staff #1 would yell loudly at the children on a daily basis.
Plan of Correction: Staff #1 has been terminated.
Standard 8VAC20-780-400-C
When time out is used as a behavior guidance technique: it shall be used sparingly and shall not exceed one minute for each year of the child's age.

Staff #3 and #4 reported that Staff #1 did not use time out sparingly and exceeded more than one minute for each year of the child's age. Staff #4 reported that Child#2, 3 years old, was placed in time out on the blue couch or the chair near the desk for more than 10 minutes. This would happen daily. Staff #3 reported that Child #3, 3 years old, was placed into timeout by Staff #1for 15 minutes.

Standard 8VAC20-780-410-1
The following actions are forbidden: Physical punishment, striking a child, roughly handling or shaking a child, restricting movement through binding or tying, forcing a child to assume an uncomfortable position or exercise as punishment.

Staff #2, 3, and 4 reported during interviews that they observed Staff #1 flipping Child #1, age 3 years old, out of a cot and onto the floor after naptime. Staff #3 and Staff #4 reported observing Staff #1 rough handling Child #2. Staff #3 reported that Staff #1 grabbed Child #2 by the upper arms and placed him on the carpet harshly. Staff #4 reported that Staff #1 picked Child #2 up from the floor by the both wrists and carrying him across the room and into the bathroom.
Plan of Correction: Staff # 1 has been terminated. Main Street Child Development Center staff will participate in re-training about CPS Reporting and Forbidden actions.
