Inspector Notes
A monitoring inspection was conducted on June 13, 2022. There were twenty-eight children present, ranging in ages from infants to five years, with six staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of three child records and three staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Violations
4Evidence: The records for staff # 1 (start date April 10, 2022) and staff #2 (start date April 10, 2022) did not have a completed central registry search. Facility had documentation that the forms were mailed on March 9, 2022 but there was no documentation of follow up with the central registry search unit to get an update on the status.
Evidence: The most recent fire inspection report was dated December 21, 2020, which is more than a year old.
Evidence: A disinfectant spray solution labeled ?Caution and Keep out of Reach of Children? was sitting on a table in the school age classroom where children were sitting.
Evidence: The following was found during staff record review:
1. The records for staff #1 (start date April 10, 2022) and staff #2 (start date April 10, 2022) did not have documentation of orientation training although staff report the training was completed.
2. The record for staff #3 (start date June 8, 2022) did not have documentation of two reference checks and emergency contact information.
