Inspector Notes

A monitoring inspection was initiated on February 15, 2023 and concluded on February 15, 2023. There were 19 children present with 8 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 8 child records and 5 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program

Standard 8VAC20-780-240-B
Based on record review the center did not ensure 1 of 5 staff complete the Virginia Department of Education (VDOE) sponsored orientation course within 90 calendar days of employment.

Evidence: The record for staff #5 (start date 4/10/2022) did not have documentation of completing VDOE sponsored orientation training. Director confirmed the staff is in the process of completing the training.

Based on observation and staff interview the center did not ensure cribs shall be provided for children during the designated rest period.

Evidence: The director confirmed that Child #7?s crib was put away in the closet because they sleep better in a baby bouncer/rocker.

Based on observation and staff interview the center did not ensure cribs shall be identified for use by a specific child.

Evidence: Staff confirmed that the crib for child #9, sleeping in the crib at the time, was not labeled with their name.

Based on observation and staff interview the center did not ensure there shall be at least 12 inches of space between occupied cots, beds, and rest mats.

Evidence: Children were observed during naptime. There was approximately 2-6 inches between the cots in the Toddler classroom and the cots were directly touching in the 3-4 year old classroom. Director confirmed the cots were too close.

Based on observation and staff interviews the center did not ensure for each group of children, the center shall maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurs.

Evidence: Staff in the Toddler room confirmed their attendance sheet showed 6 children checked in when there were 9 children in the classroom.
