Inspector Notes
An unannounced renewal inspection was conducted on December 11, 2023 from approximately 10:15 am to 12:10 pm. Seven staff were present supervising twenty-nine children ranging in age from infants through preschool age. Additional staff were on site for administrative and kitchen duties. Children were observed engaged in various group activities. Four staff and five child records were reviewed. Violations were found and are noted on the violation notice.
Violations
3Based on observation, facility did not ensure areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition.
Evidence: The following was observed during inspection:
1. One of three toilets in the hall bathroom is not operable with a sign "out of order" on the lid.
2. Two signs 4' x 8' were observed propped against the building and fence on the outside playground. The signs were not secured, within reach of children when playing outside and could fall on someone.
3. Two tricycles on the playground were missing a foot pedal.
4. The privacy fence on one portion of the playground did not reach the ground level (approximately 1' opening) and could allow a child to crawl out of the play area. Owner stated trees had recently been removed that had blocked the openings.
The signs and tricycles will be removed from the playground today.
This portion of the privacy fence will be replaced once weather allows. Until that time, children will not be allowed access to this portion of the playground effectively immediately.
Evidence: Two sinks in the hall bathroom did not have soap available for children.
Evidence: The following was observed:
1. On the small playground, a 16" plastic slide was observed placed directly on the ground with no resilient surfacing.
2. On the large playground, a merry-go-round and circular tricycle rider were observed with less than a 1" of resilient surfacing.
