Standard 8VAC20-780-160-C
(Repeat Violation) At least every two years from the date of the first initial screening, staff members must obtain and submit the results of a follow-up tuberculosis screening.
The most recent documentation of TB screening for staff #1 is more than 2 months overdue. The TB screening for staff #2 is more then 5 months overdue.
Plan of Correction: The program director will speak to staff #1 and staff #2 today and request that they have updated TB screenings updated within 2 weeks.

The required ratio of adult to children for children birth to 6 months old is 1 staff per 4 children.
There were six infants, ages 2 months to 1 year old with one staff in the infant room.
Plan of Correction: A second staff had been in the classroom but left briefly to handle a personal task. The staff was gone approximately 20 minutes. There were adequate staff in other classrooms to cover the ratios. In the future, staff will use the hand held radios to communicate with each other and the staff in charge and determine if another staff, not required for their group's ratios, can assist.

When centers choose to provide meals or snacks, they must follow the most recent, age-appropriate nutritional requirements of the Child and Adult Care Food Program of the United States Department of Agriculture (USDA).
The children were observed eating an afternoon snack which consisted of animal crackers and water, which only meets one food component and does not meet the requirements of USDA.
Plan of Correction: The director will remind staff to follow the posted monthly menu and to call for assistance if the correct food is not brought to their classroom for meals or snacks.
