Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A subsidy health and safety inspection was initiated on June 7, 2021 and concluded on June 8, 2021. The provider was contacted by telephone to initiate the inspection. There were 4 children present and 2 staff. A licensing inspection was conducted on the same date as the SHSI supplemental inspection. Both inspections combined cover subsidy health and safety regulations. Only subsidy regulations that are not covered in licensing standards are reviewed during this supplemental inspection.

The information gathered during the inspection determined non-compliance(s) with subsidy regulations, and violations were documented on the violation notice issued to the facility.

Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. You will need to specify how the deficient practice will be or has been corrected. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Based on a review of records on June 8, 2021, the provider did not ensure all caregivers who work directly with children and were hired prior to March 1, 2019 had completed the department's health and safety update course by the required deadline of 3/1/20.
Evidence: 1. The record of Caregiver #1 did not contain documentation of a completed Virginia Health and Safety update training.
2. The record of Caregiver #2 (DOH 9/4/18) contained documentation of a completed Virginia Health and Safety update training dated 6/8/21.
Plan of Correction: I will complete training yearly along with staff before deadline. Reminder posted on planner.
