Inspector Notes

An unannounced subsidy health and safety (SHSI) inspection was conducted on April 11, 2022. There were six children present with 2 caregivers supervising. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy health and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Based on a review of records and interview on April 11, 2022, the vendor did not maintain a written hard copy record of daily attendance that documents the arrival and departure of each child in care as it occurs.
Evidence: 1. Five children were signed in on the attendance sheet while six children were observed in attendance. Child #6 was not signed in on the attendance sheet. 2. The daily attendance sheet did not reflect the arrival time of child #2, child #4, and child #5. 3. The provider acknowledged child #6 was not signed in and that the arrival time for child #2, child #4, and child #5 was not documented.
Plan of Correction: We will supervise sign in/out every morning to ensure parents are signing and putting in the time from this point on.
