Inspector Notes
A monitoring inspection was initiated on 12/20/21 and concluded on 05/03/22 in response to a self-reported incident regarding the supervision of children. The Department was notified of the incident on December 20, 2021. On December 30, 2021, the inspector was on site from approximately 3:00pm to 3:52pm. Interviews were completed, observations were made, and documentation was reviewed. The preponderance of the evidence gathered during the investigation did not support violations of the standards. There are no violations resulting from this inspection.
