Inspector Notes

An unannounced monitoring inspection was conducted on June 17, 2024. The inspection began at 9:30am and concluded at approximately 11:50am. Children were observed playing with table activities, tummy time, playing on the playground, and participating in circle time activities. There were 61 children present, ranging in ages from 4 months to 7 years, with 14 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 6 child records and 6 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s).

Standard 8VAC20-780-270-A
Based on observations, areas, and equipment inside and outside the center were not maintained in a clean, safe, and operable condition.

Evidence:
1. In the infant II classroom, there were soft blocks that were torn on the edges exposing the inside materials.
2. In the 4?s classroom, there was a blue plastic child size chair with a crack in the seat measuring 4-6 inches that can pinch, or scratch skin and snag clothing. There was a rug that had fraying edges, posing a trip hazard to the children.

Based on observation, the center did not ensure to maintain, in a way that is accessible to all staff who work with children, a current written list of all children's allergies, sensitivities, and dietary restrictions documented in the allergy plan required in 8VAC20-780-60 A 8. This list shall be dated and kept confidential in each room or area where children are present.
