Inspector Notes

An unannounced inspection was initiated and completed on December 3, 2024, in response to a complaint received by the licensing office on November 23, 2024, relating to allegations of care and protection. The inspector reviewed 1 child?s record and 1 staff records on-site. This inspection included interviews, document review, and observations.

The preponderance of evidence gathered during the investigation supports the allegation(s); therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 12/20/2024. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-340-A
When staff are supervising children, they shall always ensure their care, protection, and guidance.

1. On November 21, 2024, staff #1 was supervising six children, the youngest child in the group was 13 months of age. Staff #1 was reading a story with three of the children, while three other children were playing. Child #1 began to cry. Staff #1 asked child #1 to ?come here? which was when Staff #1 noticed a braid hanging from child #1?s scalp. Staff #1 immediately called for assistance, and she held and consoled child #1.
2. Staff #1 stated that she did not know how child #1?s braid became detached from Child # 1?s scalp.
Plan of Correction: Staff will be trained in the proper way to handle any child's injury, even if no visible trauma is present. Always provide an icepack as a precaution. Staff will continue to provide comfort for the child, as apart of standard practice.

The required ratio for children ages birth ? 16 months is one staff per four children.

On November 21, 2024, in the Infant II classroom there was 1 staff with 6 children from approximately 8:30am ? 10:03am. The youngest child was 13 months.
Plan of Correction: Corrective action is to always access ratios, have a clear plan, staff training as it pertains to ratio guidelines, and regular monitoring to ensure compliance throughout the day.
