Inspector Notes

An unannounced monitoring inspection was conducted on November 8, 2023, from 11:40am to 12:36pm with the provider. There were two children, ages 3 and 4 years old, were in the care of one caregiver for a total of 4 points. The children were observed playing with age-appropriate toys. Two children?s records were reviewed. The provider and one household member?s records were reviewed and in compliance. The provider has current certification in CPR and First Aid. The first aid kit, flashlight, and battery-operated radio were observed. The emergency practice drill log was reviewed and in compliance.

Based on a review of two children's records, the provider did not obtain documentation of a physical examination for all children enrolled within 30 days after their first day of attendance. Evidence: Child B's (start date 5/5/2020) record was missing documentation of a physical examination.
Plan of Correction: The provider will request the missing information from the parent and update the child's record.

Based on a review of two children's records, the provider did not ensure that all the children's records contained the required information. Evidence: Child A's record was missing the name, office address and telephone number of the child's physician and medical insurance information.
