Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, document review and a virtual tour of the program.

A monitoring inspection was initiated on 08/18/2021 and concluded on 08/31/2021. The provider was contacted by telephone and a virtual inspection was conducted. There were 2 children, both were 2 years of age, and 1 provider present during the inspection. The inspector reviewed compliance in the areas of personnel, caregiver training, physical environment and equipment, emergencies, nutrition and background checks. A total of 2 children?s records and 1 provider record were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Standard 22VAC40-111-140-B-3
Based on document review and discussion, the provider failed to obtain current certification in cardiopulmonary resuscitation (CPR), as appropriate to the age of the children in care, from the American Red Cross, American Heart Association, American Safety and Health Institute, or the National Safety Council, or current CPR certification issued within the past two years by a community college, a hospital, a rescue squad, or a fire department.

Evidence: The most recent CPR certification obtained by the provider was not issued by an approved organization; the CPR certification was issued by the National CPRFoundation.
Plan of Correction: Will enroll in an approved class.
Standard 22VAC40-111-500-F
Based on observation and discussion, the provider failed to ensure that trampolines shall not be used during the hours children are in care.

Evidence: There was a mini-trampoline in the outside play area in the front yard. The provider stated that the children had used the trampoline.
Plan of Correction: Will get rid of the trampoline.

Based on record review and discussion, the provider failed to ensure that an annual review of emergency contact information required by the standards occurs with the parents and to obtain the parent's signed acknowledgement of the review.

Evidence:
1. The record for Child 1 did not contain verification that an annual review of emergency contact information took place with the parent; the date of the most recent review was 07/26/2020.
2. The record for Child 2 did not contain verification that an annual review of emergency contact information took place with the parent, the date of the most recent review was 10/03/2019.
Plan of Correction: Will set up a time to get all information updated.
