Inspector Notes

An unannounced renewal inspection was conducted on 03/29/2022. There were 3 children, ages 6 months-3 years, totaling 10 points, and 1 provider present during the inspection. The inspector reviewed compliance in the areas of personnel, caregiver training, physical environment and equipment, emergencies, nutrition and background checks. A total of 3 children?s records and 1 provider record were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Standard 8VAC20-800-60-B
Based on record review and discussion, the provider failed to ensure that the record for each child enrolled contains all of the elements as required by the standard.

Evidence:
1. The record for Child 1 did not contain the following information: office address and telephone number of the child?s physician; name, address and telephone number of two designated person to contact if case of an emergency if the parent cannot be reached (there was not an addresses provided for the second contact); and the names and addresses of previously attended child day care and schools.
2. The record for Child 2 did not contain the parent?s signed acknowledgement of the receipt of the information required by 8VAC20-800-70.
Plan of Correction: Will get parents to complete this evening.
