Inspector Notes

An unannounced monitoring inspection was conducted on 09/13/2023. There were 3 children, ages 22 months-2 years, totaling 8 points, and 1 provider present during the inspection. The inspector reviewed compliance in the areas of personnel, caregiver training, physical environment and equipment, emergencies, nutrition and background checks. A total of 5 children?s records and 1 provider record were reviewed. The inspector discussed potty training and school-age care with the provider. The inspector arrived for the inspection at 8:05 am and departed at 9:25 am.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Standard 8VAC20-800-60-B
Based on record review, the provider failed to ensure that a child?s record shall contain all of the elements as required by the standards.

Evidence:
1. The record for Child 1 did not contain the following information: name, address, and telephone number of each custodial parent?s place of employment (the telephone number for the place of employment listed for the 2nd parent was not provided).
2. The record for Child 2 did not contain the following information: name, address, and telephone number of each custodial parent?s place of employment (the telephone number for the place of employment for the 1st parent listed was not provided); and name, address and telephone number of two designated person to contact in case of an emergency if the parent cannot be reached (the 2nd designated person listed was Child 2?s parent).
3. The record for Child 3 did not contain the following information: and name, address and telephone number of two designated person to contact in case of an emergency if the parent cannot be reached (the address for the 1st designated person listed was incomplete).
Plan of Correction: Will get the information from parents today.

Based on observations, the provider failed to ensure that children shall not be allowed to eat or drink while walking, running, playing, lying down, or riding in vehicles.

Evidence:
1. Child 1 was observed walking around while drinking from a cup.
2. Child 2 was observed walking around while drinking from a bottle.
Plan of Correction: Will take the cups/bottles while they are walking and playing.
