Inspector Notes

An unannounced renewal inspection was conducted on 03/27/2024. There were 5 children, ages 18 months-3 years, totaling 11 points, and 1 provider present during the inspection. The inspector reviewed compliance in the areas of personnel, caregiver training, physical environment and equipment, emergencies, nutrition and background checks. A total of 5 children?s records and 1 provider record were reviewed. The inspector discussed safe sleep with the provider. The inspector arrived for the inspection at 8:40 am and departed at 10:05 am.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Standard 8VAC20-800-530-G
Based on observations and interviews, the provider failed to ensure that children under 2 years of age shall not use pillows or filled comforters.

Evidence: There was a pillow observed in a crib. Provider 1 stated that Child 3 (18 months old) uses the pillow in the crib.
Plan of Correction: The pillow was removed and will no longer be used.
Standard 8VAC20-800-690-L
Based on observations, the provider failed to ensure that when toilet chairs are used, they shall be emptied promptly, cleaned and sanitized after each use.

Evidence: The training urinal in the restroom was observed with urine in it; it had not been promptly emptied, cleaned and sanitized after it was used.
Plan of Correction: The urinal was emptied during the inspection and it will be cleaned and sanitized as soon as possible.
Standard 8VAC20-800-780-B
Based on record review, the provider failed to obtain the parent?s signed acknowledgement of reviewing the child?s emergency contact information annually.

Evidence:
1. The most recent signed acknowledgement of the review of the emergency contact information in the record for Child 1 was obtained on 02/01/2023; an annual update was not obtained by 02/01/2024.
2. The most recent signed acknowledgement of the review of the emergency contact information in the record for Child 2 was obtained on 12/07/2022; an annual update was not obtained by 12/07/2023.
Plan of Correction: Will get it updated today.
