Inspector Notes

A subsidy health and safety inspection was conducted on 03/27/2024. There were 5 children, ages 18 months-3 years, totaling 11 points, and 1 provider present during the inspection. The inspector reviewed compliance in the areas of personnel, caregiver training, physical environment and equipment, emergencies, nutrition and background checks. A total of 5 children?s records and 1 provider record were reviewed. The inspector discussed safe sleep with the provider. The inspector arrived for the inspection at 8:40 am and departed at 10:05 am.

A licensing inspection was also conducted on this date. Both inspections combined cover health and safety regulations. Only subsidy requirements not cover in licensing standards were reviewed during this supplemental inspection.

Information gathered during the inspection determined non-compliance with applicable subsidy requirements and violations were documented on the violation notice issued to the provider.

Based on record review, the vendor failed to maintain a written hard copy record of daily attendance that documents the arrival and departure of each child in care as it occurs.

Evidence: The attendance record for 03/26/2024 did not document the children?s departure. There was a total of 5 children observed in attendance during the 03/27/2024 inspection. The attendance record did not document any children in care; the attendance was not taken upon arrival as it occurred on 03/27/2024.
