Inspector Notes

An unannounced on-site monitoring inspection was conducted on 03/21/2025. The on-site inspection began at 9:45 am and ended at 10:45 am. The inspector reviewed compliance in the areas listed above. There were 5 children present and 1 caregiver; the point total was 12. The inspector reviewed 5 children?s records and one caregiver record.

The inspection included document review, tour of the home, interviews and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Before a child may attends the family day home, the provider shall obtain documentation that the child has been adequately immunized.

The immunization record for Child 3 was obtained 3 weeks after the first day of attendance.
Plan of Correction: Will get the information prior to starting.
