Inspector Notes

An unannounced on-site monitoring inspection was conducted on 09/22/2025. The on-site inspection began at 9:10 am and ended at 10:15 am. The inspector reviewed compliance in the areas listed above. There were 3 children present and 1 caregiver; the point total was 10. The inspector reviewed 5 children?s records and a caregiver record. The inspection included document review, tour of the home, interviews and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
