Inspector Notes
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A subsidy health and safety inspection was initiated on June 14, 2021 and concluded on June 20, 2021. The provider was contacted by telephone to initiate the inspection. There were 45 children present and nine staff. A licensing inspection was conducted on the same date as the SHSI supplemental inspection. Both inspections combined cover subsidy health and safety regulations. Only subsidy regulations that are not covered in licensing standards are reviewed during this supplemental inspection.
The information gathered during the inspection determined non-compliance(s) with subsidy regulations, and violations were documented on the violation notice issued to the facility.
Violations
1Evidence:
1. The record for staff #1 and staff #2 had documentation of completing the annual health and safety training on 11/01/19.
2. The record for staff #3 had documentation of completing the annual health and safety training on 9/29/19.
3. Completion of the Annual Virginia Health and Safety Update training for child care staff is required annually of subsidy vendors and their staff.
