Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was initiated on June 14, 2021 and concluded on June 20, 2021. The provider was contacted by telephone to initiate the inspection. There were 45 children present and nine staff. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed three children?s records and three staff records submitted by the facility to ensure documentation was complete.

Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.

Standard 22VAC40-185-160-C
Based on a review of staff records, the center did not ensure that subsequent communicable tuberculosis (TB) screenings were conducted at least every two years from the date of the initial screening.

Evidence:
1. The record for staff #1 (date of last TB screening/test 03/27/18) did not contain updated TB screening results as of 06/20/21.
2. The record for staff #2 (date of last TB screening/test 6/13/19) did not contain updated TB screening results as of 06/20/21.
Plan of Correction: Staff #1 had a chest xray on Monday 6/22/21 and a copy was sent. Staff #2 is pregnant she will be getting a letter from her doctor by 6/29/21 and I will fax a copy of that as well.I will be reviewing all child and employee files to ensure that everything is up to date. Debbie and Tracy Marco are responsibile.
Standard 22VAC40-191-60-C-2
Based on record review, the center did not ensure that of one of three staff records contained documentation of central registry findings within 30 days of employment.

Evidence:
1. The record for staff #3 (date of hire 3/18/18) did not contain a central registry finding.
2. Administrator #1 stated that she could not locate the central registry for staff #3.
Plan of Correction: Staff #3 is currently on vacation. Her paperwork is ready as soon as she comes back to be notized and sent off for update. I will be reviewing all child and employee files files to ensure everything is up to date. Debbie and Tracy Marco are responsible.
