Inspector Notes
An unannounced monitoring inspection was conducted on April 18th, 2024. Two inspectors were on site from approximately 9:45am until 1:30pm. There were 28 children present with 7 staff supervising. There was a total of four classrooms visited, ranging in ages from infants to preschool. Children were observed in variety of activities including interacting with staff while doing art activities, playing with building blocks, reading, singing songs, coloring, eating lunch. Inspectors interacted with staff and children in care. If you have questions regarding this inspection, please contact the Licensing inspector.
The inspectors reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of six child records and six staff records were reviewed.

Violations
10Evidence: The record of child #3 (DOE 12/13/23) contained an immunization record date 3/14/24.
Evidence:
1. Child #4 (DOE 3/16/21) had one immunization record dated 2/26/21.
2. Child #6 (DOE 4/26/21) had one immunization record dated 4/23/21 and a late immunization update dated 2/18/22.
3. Child #5 (DOE 7/17/23) had one immunization record dated 6/26/23 and a late immunization update dated 3/6/24.
Evidence: The record for Staff #6 had documentation of an orientation training that did not include prevention of sudden infant death syndrome and use of safe sleep practices, prevention of shaken baby syndrome and abusive head trauma.
Evidence:
1. There was a total of 4 sets of coat hooks hanging from the fences outside that were all rusted.
2. A broken chair was found with multiple cracks that could entangle clothes or pinch skin.
Evidence: 7 out of 7 booster seats in the center?s vehicle were expired. The expiration dates ranged from December 2010-December 2013.
Evidence: A child from the preschool room was observed walking unsupervised to another classroom to use the restroom.
Evidence:
1. The infant room play space indoors did not have any equipment and toys that support large motor development.
2. The outside play area for infant room did not have any equipment and toys that support large motor development.
Evidence:
1. Staff #6 was observed putting on gloves to change an infant diaper without washing hands, changed the infant diaper, and did not wash hands after diapering the child.
2. In the infant room, the hands of two children were not washed after their diapers were changed.
Evidence: Staff #6 did not sanitize the diapering surface after the surface was cleaned.
Evidence: Child #4 and Child #6 did not contain documentation of child updates and confirmation of up-to-date information in the child?s record as required by 8VAC20-780-420-E-3.
