During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes
An unannounced monitoring inspection was initiated on 05/07/2025 and completed on 05/21/2025 in response to a self-report received by the licensing office on 05/05/2025 relating to injury and transportation. The inspector reviewed one child?s record and 10 staff records on-site. This inspection included document review, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Though not a part of the original self report, violations that were not reported were found related to administration and are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on (date). A POC submitted after this date will not appear on the public website.
Violations
5Staff #1, staff #2, staff #3, staff #4, staff #5, staff #6, staff #8, staff #9, staff #10 was not provided in writing precautions in transporting children.
On 05/05/2025, staff #7 was driving the center?s van to pick up children from school. The van was not put in park. The van?s door was open and the van went into reverse and bumped child #1 to the ground. First aid was administered to child #1.
The center did not provide in writing the center?s transportation policy for child #1.
Bus #1, that is used daily to transport children, had a state inspection that expired seven days ago.
On 05/05/2025, the center transported child #1 and did not have parental permission to transport. The center transports children to and from school daily.
