An announced investigation was was initiated on 5/21/2021 regarding a self-reported incident that occurred at the center on 5/20/2021. The inspection was concluded on 5/26/2021. Interviews were conducted and documentation was reviewed. After carefully interviewing all staff involved, it was determined that there was non-compliance with applicable standards or laws and violations were documented on the violation notice issued to the facility. Based on interview, children under the age of 10 were not kept within actual sight and sound supervision of staff.
Evidence: On 5/20/2021 at approximately 5:00pm, Child #1 (30 months old) walked off the toddler playground and onto the school aged playground through a gate that connected the two playgrounds. Staff #1 estimates that Child #1 was missing off the toddler playground for approximately 1 or 2 minutes. Child #1 was found sitting by themselves at the picnic table on the school aged playground.

Plan of Correction: Place bungee cord around the interior fence latches to prevent children from opening gates on their own. Review playground safety will all staff by 5/28/2021.
