Inspector Notes

Today an unannounced monitoring Inspection took place today. There were 10 classrooms observed with a total of 107 children with 18 staff within the supervision guidelines. The children were observed using stencils, playing with magnatiles, painting a volcano and other organized activities. A complete inspection of the physical plant, children and staff records, fire drill log, medication and emergency procedures were observed during this inspection. There was an abundant supply of materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. Areas of non-compliance are identified on the Violation Notice.

Based on record review, the center did not obtain additional immunizations once every six months for all children under the age of two years.
Evidence: Child #4 (19 months) most recent documented immunizations are from when the child was 6 months old.
Plan of Correction: The child's immunizations are up to date and their records are now on file.

Based on observation, equipment in the center was not maintained in a clean, safe and operable condition.
Evidence:
1. The play floor mat and climbing blocks in the older infant classroom is frayed and torn.
2. The blue childrens chair and couch in the Toddler A classroom are frayed and torn.
3. The blue childrens chairs in the Discovery Preschool A classroom are frayed and torn.
Plan of Correction: New items have been ordered to replace these items. Old items will be removed as soon as new ones arrive.

Based on observation and interview, cots were not identified for use by specific children.
Evidence: The cots in the Discovery Preschool A and Pre-K classrooms are not labeled for identification.
Plan of Correction: Cots in all classrooms are all labeled for individual use.
