During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced monitoring inspection was initiated on 9/16/2024 and completed on 9/16/2024 in response to a self-report received by the licensing office on 9/12/2024 relating to staffing and supervision. The inspector reviewed 2 children?s records and 3 staff records on-site on 9/16/2024 and electronically on 9/16/2024. This inspection included document review, tour of the facility, and interviews.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 10/09/2024. A POC submitted after this date will not appear on the public website

(Repeat violation) (Systemic deficiency)
When staff are supervising children, they shall always ensure their care, protection, and guidance.

On 9/11/2024, Child #1, 3 years old, exited the playground and entered an empty classroom to use the restroom without the awareness of any of the three staff, counted in the staff-to-children ratios, who were outside with children on the playground. Child #1 was later found inside the PreK A classroom by a parent, alone and crying. Child #1 was reportedly panicking because she was unable to open the door to exit.
Plan of Correction: All staff were retrained on Active Supervision and Child Supervision Recording Procedures before 09/30/2024. Locks and door chimes have been ordered and will be installed on all exterior doors to prevent accidental re-entry and to alert staff if doors are open. Management will complete spot checks in all classes at least once per day to ensure active supervision is taking place. All classes will be observed by a manager at least once per month in which transitions and active supervision will be observed. All center staff will participate in monthly training which will be based on information gathered during management observations of active supervision and transitions.

Children under 10 years of age shall always be within actual sight and sound supervision of staff.

On 9/11/2024, Child #1, 3 years old, was out of sight and sound supervision for an estimated four to seven minutes. A parent found Child #1 inside the PreK A classroom, alone, and crying while the child?s class was outside on the playground.
