During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site renewal inspection was initiated on 9/22/25 and completed on 9/22/25, as a part of the provisional licensure period. The on-site inspection began at 9:40am and ended at 11:55am. The inspector reviewed compliance in the areas listed above. There were 76 children present and 14 staff. The inspector reviewed 8 children?s records and 5 staff records on-site. This inspection included document review and a tour of the facility.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

The center is required to obtain an out of state sex offender registry search from any state in which the individual has resided in the preceding five years.

The center did not request a sex offender registry search from one state for Staff #3, who has been employed for 182 days and was working with children at the time of the inspection.
Plan of Correction: Completed the out of state sex offender registry search during the inspection.
Standard 8VAC20-780-140-A
Physical examinations completed by a physician shall be obtained prior to the first day of attendance or within 30 days after the first day of attendance.

There were no physical examinations in the records of Child #3, who had been enrolled for 57 days and Child #6, who had been enrolled for 84 days.
Plan of Correction: Will request and obtain the missing health forms from the parents.
Standard 8VAC20-780-70
Staff records must have documentation that two or more references as to character and reputation as well as competency were checked before employment.

Both reference checks for Staff #3 were completed after employment: the first six days and the second seven days after employment.
Plan of Correction: Will ensure to complete reference checks prior to hire date going forward.
