Inspector Notes
An unannounced monitoring inspection was initiated on 12/03/2025 and completed on 12/12/2025 in response to a self-report received by the licensing office on 11/21/2025 relating to supervision of children and behavioral guidance. The inspector reviewed one child's record and three staff records on-site. This inspection included document review, tour of the facility, and interviews.
The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
