The green chair in the Pre-K A classroom and the blue chair in the Preschool A classroom contained numerous tears in the seating material. In the two-year old classroom, peeling paint is present on the wall near the classroom entrance.

Staff shall wash hands with soap and running water before and after a diaper change. Children?s hands shall be washed with soap and running water after toileting, or any contact with blood, feces or urine.
Staff # 2 changed child A's diaper in the two year old classroom and did not wash the child's hands with soap and water. Staff #2 did not wash hands before or after changing child A's diaper and continued to assist child B with diapering.
