An unannounced complaint investigation was conducted on 12/13/22 from 2:30pm to 3:15pm with the Director and Owner in response to a complaint received regarding allegations related to preventing the spread of disease. Based on review of documentation, policies and procedures, and interview, the allegations were unable to be substantiated due to insufficient corroborating evidence.
