Inspector Notes

An unannounced inspection was initiated on 07/30/24 and completed on 07/30/24 in response to a complaint received by the licensing office on 07/18/24 relating to allegations of behavioral guidance and injury reporting. The inspector reviewed one child?s records on-site. This inspection included document review, tour of the facility, interviews, and observations.

The preponderance of evidence gathered during the investigation supports a part of the allegations; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 10/07/24. A POC submitted after this date will not appear on the public website.

The center is to maintain a written record of children's serious and minor injuries in which entries are made the day of occurrence.

Child #1 sustained a minor injury at the center on or about 07/16/24, but a written record of the child's injury was not documented.
Plan of Correction: Per the Center: "Retrained staff on 7/18/2024 any accident with or without visible mark must be documented in a written record. Reiterated importance of documentation of minor injury at our monthly staff meeting."
