Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews, and a virtual tour of the program.

A monitoring inspection was initiated on November 1, 2021 and concluded on November 3, 2021. The director was contacted by telephone and a virtual inspection was conducted. There were 58 children present, ranging in ages from 22 months to six years of age, with eight staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records and five staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on review of staff records, the center failed to obtain required out of state background checks in the required time frame.

Evidence:
1. Staff #2, date of employment 09/08/2021, indicated on the sworn disclosure statement dated 08/13/2021 that they had resided in two states outside of Virginia within the past five years.
2. A criminal history check was required before the date of employment. One criminal history check was obtained on 10/07/2020, and there was no documentation of a criminal history check being completed for the other state.
3. An out of state central registry finding is required to be sent off within 30 days of employment. The out of state central registry findings had not been completed for either state. The requests should have been made by 10/08/2021.
Plan of Correction: Administration misunderstood the requirement. They will complete all required out of state background checks immediately. They will send proof that the background check or has been completed to the licensing inspector within 10 business days.
