Inspector Notes

A renewal inspection was conducted on March 15, 2023 from 2:00 P.M.-4:15 P.M. There were 27 children present, ranging in ages from two to six years of age, with five staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of seven child records and six staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on a review of children records, the provider failed to ensure a physical examination was on file before the child's first day of attendance or within 30 days after the first date of attendance.

Evidence:
1. Child #4's date of enrollment was 08/14/2022. The physical examination on file was dated 11/21/2022.
2. Staff #1 verified the date of enrollment and the date on the physical examination.

REPEAT VIOLATION
Based on review of staff records, the center failed to ensure that the Virginia Department of Education sponsored orientation course was completed within 90 calendar days of employment.

Evidence:
1. Staff #2's date of employment was 10/12/2022. The training certificate was dated 01/26/2023. The training was required to be completed by 01/12/2023.
2. Staff #1 verified the date of employment and the date the training was completed.
