Inspector Notes

An unannounced monitoring inspection was started in the center on October 25, 2023 from 11:10 AM until 12:45 PM and concluded on October 25, 2023. At the time of entrance, 21 children were in care with 3 staff members and 4 parent volunteers. The sample size consisted of 5 children?s records and
5 staff records. Children were observed playing outside, during story time and share time.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 8VAC20-780-270-A
Based on observation, measurements and staff interviews, the licensee did not ensure areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition.

Evidence: In the common area, there was a refrigerator that is 2?7? high sitting on top of a 2 `8? high cabinet. The refrigerator is 2 inches deeper than the top of the cabinet. The refrigerator was not secure as it easily moved when touched and it was accessible to children in care and posed a tipping hazard.
Plan of Correction: The refrigerator was immediately moved to a new location by the Director and Assistant Director.
Standard 8VAC20-780-60-A
Based on review of 5 children?s records and staff interviews, the licensee did not ensure to maintain and keep at the center a separate record for each child enrolled which contained the required information.

Evidence: The following information is missing from 2 out of 5 of the children?s records reviewed.
Child #3 did not have the employment information for one parent listed and the employment phone number for the other parent listed.
Child #5 did not have the employment phone number for one of the parents listed.
Plan of Correction: 1) Director contacted families to verify there was not information to be filled in on files.
2) Director corrected files accordingly.
