Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, and a virtual tour of the program.
A monitoring inspection was initiated on 1/27/2022 and concluded on 1/27/2022. The director was contacted by telephone and a virtual inspection was conducted. There were 89 children present, ranging in ages from 3 months to 12 years, with 19 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 4 child records and 4 staff records were reviewed.
Information gathered during the inspection determined non-compliance with an applicable standard and the violation was documented on the violation notice issued to the program.

Based on review of four children's records and staff interview, the facility failed to have all required information in one child's file. Evidence: The record for child 1 (date of enrollment 9/8/2020) does not contain documentation of proof of identity. The proof of identity had been reviewed, but the documentation could not be located.
Plan of Correction: Center staff will obtain documentation of proof of identity as soon as possible.
