Inspector Notes

An unannounced monitoring inspection was conducted on-site on 11/07/2022 and an addition record review was conducted on 11/10/2022. There were 6 children, ages 3 years- 4 years, and 2 staff members providing direct supervision present during the inspection. The inspector reviewed compliance in the areas of administration, staff qualifications and trainings, physical plant, staffing and supervision, programs, special care provisions and emergencies, special services, and background checks. The children were observed during child-choice activities, teacher-directed activities and outdoor time. A total of 5 children?s records, and 2 staff records were reviewed. The inspector discussed the following with the provider: physical plant and sanitation report. The inspector arrived for the onsite inspection at 9:30 am and departed at 11:15 am and arrived for the record review at 11:15 am and departed at 12:00.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Based on record review, the center failed to ensure that the separate record for each enrolled child shall contain all of the elements as required by the standards.
Evidence:
1. The record for Child 1 did not contain the following information: documentation of first day of attendance.
2. The record for Child 2 did not contain the following information: documentation of viewing proof of the child?s identity and age.
3. The record for Child 3 did not contain the following information: documentation of first day of attendance.
4. The record for Child 4 did not contain the following information: documentation of viewing proof of the child?s identity and age and documentation of first day of attendance.
5. The record for Child 5 did not contain the following information: documentation of chronic physical problems and pertinent developmental information and any special accommodation needed, documentation of previous child day care and schools attended by the child documentation of viewing proof of the child?s identity and age and documentation of first day of attendance.
