Inspector Notes
An unannounced renewal inspection was conducted onsite on 02/14/2023 and concluded with an electronic record review on 02/15/2023. There were 10 children, ages 3 years -5 years, and 2 staff members providing direct supervision present during the inspection. The inspector reviewed compliance in the areas of administration, staff qualifications and trainings, physical plant, staffing and supervision, programs, special care provisions and emergencies and special services. The children were observed during mealtime, teacher directed activities and outdoor time. The inspector discussed the following with the provider: playground, children?s records, and lockdowns. A total of 5 children?s records and 2 staff records were reviewed. The inspector arrived for the inspection at 9:15 am and departed at 11:00 am.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.
Violations
2Evidence: The record for Child 2 did not document the first date of attendance; however, Staff 1 confirmed a first date of attendance as 01/04/2023. The physical was not available.
Based on record review, the center failed to ensure the separate record for each enrolled child shall contain all of the elements as required by the standards.
Evidence:
1. The record for Child 1 did not contain the following information: chronic physical problems and pertinent developmental information and any special accommodations needed, and work phone number for parent 1.
2. The record for Child 2 did not contain the following information: first date of attendance.
3. The record for Child 3 did not contain the following information: chronic physical problems and pertinent developmental information and any special accommodations needed, work phone number for parent 1, and allergies and intolerance to medication or any other substances and action to take in an emergency situation.
4. The record for Child 4 did not contain the following information: documentation of viewing proof of child?s identity and age.
5. The record for Child 5 did not contain the following information: chronic physical problems and pertinent developmental information and any special accommodations needed, and previous child day care and schools attended by the child, phone number for parent 1 and work phone number for parent 2.
