During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 10/09/2024 and completed on 10/10/2024. The on-site inspection began at 11:40 am and ended at 12:20 pm. The inspector reviewed compliance in the areas listed above. There were 11 children present and 2 staff. The inspector reviewed 5 children?s records onsite and 2 staff records electronically on 10/10/2024.

This inspection included document review, tour of the facility, interviews, observations, and measurements.

Standard 8VAC20-780-130-A
The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.

There was no immunization in the record of Child #3, who had been enrolled for approximately two months.
Plan of Correction: Staff will contact admin staff to obtain.
Standard 8VAC20-780-140-A
REPEAT VIOLATION
Physical examinations completed by a physician shall be obtained prior to the first day of attendance or within 30 days after the first day of attendance.

The physical examination in the record of Child #1, who had been enrolled for approximately two months, was obtained approximately 3 weeks after the due date.
