Inspector Notes
An unannounced renewal inspection was conducted today from 8:55am-11:05am. There were 26 children directly supervised by 7 staff and all classrooms were in ratio. The physical plant, outdoor playground area, programming, menus, 3 staff records, 3 children?s records, evacuation drills, injury reports, emergency supplies, and policies were inspected. No medications were on site. Children were observed participating in circle time, engaging in free play, building with blocks, finishing breakfast, and working on a holiday craft project. Positive interactions between children and staff were observed during this inspection. Areas of non-compliance are identified in the Violation Notice.

Violations
6Evidence: Staff #4 stated that Staff #1 (Date of Hire: 8/16/2021) is a rehire and updated fingerprints were not completed prior to rehiring Staff #3.
Evidence: Staff #4 stated that Staff #1 (Date of Hire: 8/16/2021) is a rehire and an updated Central Registry search was not conducted within 30 days of the staff's rehire date.
Evidence: 1. The playground surfacing fabric at the base of the slide on the smaller climbing structure has split and there was frayed fabric sticking up posing a tripping hazard. 2. There is a small hole of cracked and jagged plastic at the bottom of the larger climbing structure that could cause fingers or hands to get stuck
Evidence: The two most recent shelter in place drills were conducted 12/2/2020 and 12/3/2019.
Evidence. One bottle of prepared infant formula was observed in the refrigerator in the Infant A classroom that was not dated or labeled with the child's name.
Evidence: At approximately 9:05am, there were two children present in the Toddler A classroom and only one child was signed in on the classroom attendance sheet. Staff #2 advised that the child who was not signed in has been there since approximately 7:50am this morning.
